I strongly recommend the hardback version, as the paperback version tends to split at the binding and pages can fall out.

The Lost Chapters: The Official Addendum To: Vaseline Glass: Canary to Contemporary
Self published in 2004.  Available only from the author (see review above for email link at end of article)  Retail $15USD + postage.

When my book was originally written, I wrote enough to make a book of approximately 300 pages.  When I sent it to the publisher, he limited me to 224 pages, including index, color photos, etc.  Instead of trimming down the size of each chapter, I eliminated specific chapters (each chapter being a different glass factory).  I self published the LOST CHAPTERS in 2004, and did a full cross-referenced index, which was not in the original book.  The index in this addendum gives all names to listed patterns, not just the most commonly used names.  The book is spiral bound, 60 pages total.  Any book purchased from the author is autographed.

Included in the purchase of either book from the author is corrections that have been made since the books have been published.  The list includes items that have been identified that had not been identified at time of press or that were mis-identified (thankfully, the list is short!)
