Pyramid Shaped Blue & Green Vase - ID = Erickson

Does anyone know who made and designed this blue and green pyramid shaped vase and when it was made?  It is small and wide (4 inches/10 cm tall), with polished bottom.

Impressive sleuthing!  Fairly obscure web page to find art glass.  I knew the piece was good quality, but I'm a bit surprised at how inexpensive it is.  Erickson glass is, in my opinion, undervalued in the marketplace.
