It is a Pilgrim pitcher - they produced an astonishing range of 17 shapes in this little pitcher. The pitchers were "mouth blown and shaped by hand". The pitcher gets called "Nuggate" sometimes too. I wrote an article a few years ago about them that was published in our "Network" journal.
Date? Made betwen 1968 and 1973, at Pilgrim Glass Corporation, Ceredo (now out of business I believe).
The little pitchers (they also made a vase...ie the pitcher without a handle) are blue, with a golden iridescence. The applied handles are always orange/gold.
