Topic: Is this vase Sklo Union? - ID = Vladislav Urban for Heřmanova huť

I'm pretty sure this is a Sklo Union vase, but can't find the form anywhere. Can anyone confirm? It's about 5 3/4" tall.

It certainly is...  Designed by Vladislav Urban in 1962, Heřmanova huť, pattern no 20064.
