Small vase ID needed - ID = Glassform

Here is a small vase 4.5" - well made in deep red fading to light at top - 'water splash' type opening to top - the signature is clear and I can read most of it but it doesnt scan - think it says 69 for year - any help appreciated

Yes it does say Glass Form - thanks, it's Glass Form I think rather than glasform - seperate company http://glassform.eu/products.php?cat=27
