I think this candleholder is called 'The cube'
I'll try and find a link for you.
link to the archive studio glass they used to make.  I'm not sure it's all on there but there are some pieces.
I think he is now concentrating on lighting only???

Adrian is based in Cumbria, he used to have a studio in Ambleside where you could go watch them make all sorts of lovely things, but that closed in 2006 and they now only make lighting, and there are no visits to be had sadly. The news release for this is here: http://www.glassmakers.co.uk/news/

Adrian has thoughtfully kept his catalogue online as an archive resource (hurrah! would that others would do the same!) and this candleholder is shown here: http://www.glassmakers.co.uk/archive/displayproduct.cfm?productid=79. It is shown as made in four colours, but the green ones come in two differing shades as I have one of each, both bought from Adrian with labels: http://yobunny.org.uk/gallery1/displayimage.php?pos=-8
