Paul, heights are not given in the catalogue for caddys. The images I have come from the Winterthur Library Delaware, who will copy up to a maximum of 20 pages for a fee. However they are supposedly putting all their glass catalogues online this year at the "Internet Archive" website. The catalogue is from 1881 but their copy was annotated in 1902 with some designs being crossed out in pen, which I guess means that some were no longer produced. None of the caddys were crossed out so I would say they were still being produced in the early 20th century. Then of course the molds could have been sold on when P&V closed before WW1 - perhaps Molineaux Webb across the other side of the canal might have taken them and kept that model going for another 20 years... one can never know!

thanks Neil  -  in fact I had sent another very short note re your offer of sending the images, just saying thanks and would indeed appreciate seeing these  -  but my words seem to have been lost.     
Fingers crossed then that the full catalogue goes on line some time this year, which will no doubt help us all.

The example Julie pointed us to is pressed not cut.  My guess is that the caddy is lead glass which will give it a same bell-like ring.
