Height: 37 cm
From above: 23 cm (widest), 19 cm (narrowest)
From the base: 10 cm
Weight: 2.284 kg

Hello and welcome.  :)
I have seen vases of similar shape and quality, made by Tarnowiec. That might be a good name to start with, given your location.  ;D
I do not know much about Tarnowiec. I bought a couple of pieces, perhaps ~ 18 years ago, and I know they are lovely quality.

I have done an internet image search and I can find vases the same shape as yours and others with similar decor. I think Tarnowiec is very likely.  :)
