Please can anyone of you help I.D my glass??

It around 10 Inches tall

Please if you can any links to info and the following:

1.Maker and birth place
2.Age and background of designer
3.value and pattern name or number

Please help guys baffled!!!!

Hi, your vase is Czech, designed by Pavel Hlava for Crystalex 1969.
