Any idea about manufacturer, country or year?

Glass with that pale blue tinge and very thick walls usually came from Sweden.  However some factories made blanks that smaller factories then decorated - in your case it has had flat surfaces ground.  It probably had a paper label which has been removed by the original owner.  If you are lucky someone will recognise it, but I would not hold my breath.
