Fostoria Glass Co., Moundsville, West Virginia, USA. This company made a series of "crown" patterns, and this is one of them. Your item is likely 1950s-1960s. Fenton Art Glass acquired many of these moulds. Check the web site for Replacements, Ltd. and do a search: www.replacements.com
