Probably not Karlin Rushbrooke.

How about first name Keith or Kevin, second name is much trickier, something ***hall, it looks like Jushall but...

Hi John,

I think the first letter of the surname is an R, with an elongated cursive foot, and the last three, as you say, is ....all. The first looks like Keri? but could just as well be Kevin, Keith....... I have been through the huge list that was posted the other day.
'tis a conundrum.  ::)

I am pretty sure the first name is Kevin as I played with some colors EFFECTS  on it......

The last name looks like something like:
JUSHULL..... OR JURHULL.......JUSHALL....... JURHALL

There are just guessing fiddling with Effects on it!!

 And the guilty party is ......... drum roll ..........

Kim Birchall  :P

I tried so many combinations of names in google and eventually found this lady associated with glass and went to her on Facebook page.  :o
She was very kind and replied to my message and confirmed that it is her work. It was made 17 years ago and would have originally had a flower stopper, even though the bottle shows no signs of having had one.
