I had assumed that this was Czech, and would be an easy find for factory/designer... Chřibská, probably... Hospodka, perhaps... it'll be in a book somewhere... or online...
But I'm stumped.
It's 13cm tall, 19cm wide, moulded foot, polished base, hotworked opalescent rim, and a lovely grid of lines of teeny-tiny bubbles that terminate in much bigger bubbles.
Any pointers in the right direction gratefully received.

Oh, excellent. Thanks so much.
Incredibly frustrating, though, as I had already looked through that catalogue! I must have somehow skipped over it.
Also pops up in a 1977 issue of Glass Review: https://photos.google.com/share/AF1QipN_r-shuJ8TFYg3CkeO8GD73Lartbd-7alS3SsNzeuCx7t_ZSWakdVM_PTpb5Nz3Q/photo/AF1QipNHa8SVnolBDGUlG8OQ3A1G632ZdbVOrraAhMkj?key=NFpxS1pnQjFscmd1cG5wOGtHX1JRQ25Hc09FblV3
