Clear glass block vase

This lovely little vase has been donated to us for sale to raise funds for the hospice I work at. I wonder if someone might be able to help me identify it please. It is 5.5 inches tall

The vase is a Sklo Union piece, designed by Vladislav Urban c.1967. A quick search should reveal more examples...
