Help identifying whiskey decanter

Hello,

I wonder if is possible to get help identifying a whiskey decanter. We broke it and would like to get it replaced. Here is some info on the bottle.

It is approx. 10 inches in height with stopper.
Clear, heavy walled.
Square base and top of stopper
Bottle and stopper taper in towards neck
No markings except for a "V" on the bottom of stopper
No cuts but corners are beveled
I also attached a picture.
