Someone somewhere identified this for me once.....found one with an intact label....and I put the vase away and forgot everything.   Might be Polish, Czech Republic, Japanese, etc.  Help, please.

It's Japanese. 1960s, by Marti Glass (also known as Multi Glass or Fukuoka Glass Company).
