Another Czech glass label

I am going through my mother-in-law's things and have found a Czech art glass piece.  I don't know how to identify it.  It has a label and signature on the back.

Any suggestions?

The signature reads "Prof. Jan Cerny"

Welcome.  It's a lovely piece. Cerny is very collectible.  The label was used in the 80' (I'll check when I get my books) to signify a piece that was being sold as art glass. It doesn't tell you what factory made it; in fact, it could have come from a few.
