Need help identifying this glass vase please

We purchased this at an estate auction this weekend and we really do not know a thing about this type of glass. I have researched online to the best of my ability and was unable to find another like it. I did find lots of Afors pieces but I don't see any record of an artist by the name Vivan anywhere on the net. Your help is greatly appreciated. I am American so I have done my best to convert the dimensions to metric.
17.5 cm Tall
9 cm across the top from flat edge to flat edge
10 cm across the top from point to point

 Hello and welcome.

According to Ivo Haanstra's glass fact file book, the mark Afors+model number+artist was used from 1946-76.  I can't quite make out the model number on your piece?  Like you, I have also drawn a blank with the name Vivan.

I will try the books tomorrow if it's still unidentified.

the number between afors and the name is 0149
