Have you looked up Walsh Walsh? 
no idea if it it even could be (so this is a complete stab in the dark) but I seem to remember them doing glass panel lighting things or something?

Hi!  I just posted about the same piece of glass.  I would love to find out where you purchased yours, how you found it etc.  If you would ever have interest in selling yours.....I am looking for one.  Thank you!!!! 

Also, I did find one in a vanity tray on Etsy.  There is a bunch of info there, but I could not verify it.  Sadly, it is way over my budget.
